Services and Products

 

Enteral Therapy

A complete line of nutrition formulas are available for oral supplements and liquid tube feeding. Nutritional assessments are conducted by a registered dietitian for each patient. Patient follow-up is provided by the Nutrition Support Team to assure safe and appropriate use of formulas. The team also provides:

  • In-home visits for patient assessment, evaluation and education as needed
  • Individualized plan for patient care and follow-up
  • Comprehensive evaluation of drug interactions by a clinical pharmacist
  • Clinical monitoring of lab values and physician updates
  • Assistance with Certificate of Medical Necessity (CMN) and insurance authorization
  • Consultations with physicians and facility staff
  • Staff education available for facilities and physician office staff.

Post-Weight Loss Surgery

A variety of nutrition supplements are available for patients who have had bariatric surgery and other procedures. These products can be purchased at one of our retail showrooms and some are also available at our Web Showroom.  We have staff with specialized certifications in weight-loss surgery support and general weight management. Medical Nutrition Therapy

Medical nutrition therapy at Great Lakes Home Healthcare Services is a one-on-one counseling session conducted by a registered dietitian. Medical nutrition therapy refers to nutrition assessments focused to identify interventions helpful during treatment of an illness, injury or disease condition. The nutrition care process can include assessment, establishment of self-management goals, development of nutrition plan, documentation, communication with the primary care provider, evaluation and reassessment, and outcomes measurements.

Patients who are diagnosed with a serious condition or disease process can benefit from medical nutrition therapy, such as:

• Severe obesity (children and adults)
• Bariatric surgery patients
• Coronary heart disease
• Hyperlipidemia
• Hypertension
• Congestive heart failure
• Pediatric failure to thrive
• Gastrointestinal disorders
• Celiac Disease
• Diabetes*
• Renal failure*

Nutrition consulting services are often not covered by insurance.

*These patients are consulted by registered dietitians who are also certified diabetes educators.
