Expert Nutrition Education and Therapy

Good nutrition is a key to maintaining your health and wellness, particularly when you experience a chronic health condition or multiple diseases.

At Great Lakes Home Healthcare Services, our Nutrition Support Team – registered dietitians, nurses, pharmacists, reimbursement specialists – offers Enteral and Total Parenteral Nutrition (TPN) expertise as well as medical nutrition therapy and supplements for post-weight loss surgery. Our services are available to patients throughout northwestern Pennsylvania and western New York.


Enteral Therapy

GLHHS offers a complete line of nutrition formulas for oral supplements and liquid tube feeding. Our registered dietitians conduct a nutritional assessment for every patient and while our Nutrition Support Team follows up to ensure safe and appropriate use of formulas. The team also provides:

  • In-home visits for patient assessment, evaluation and education
  • Individualized patient care plans
  • Comprehensive evaluation of drug interactions by a clinical pharmacist
  • Clinical monitoring of lab values and physician updates
  • Assistance with Certificate of Medical Necessity (CMN) and insurance authorization
  • Consultations with physicians and facility staff
  • Staff education for facilities and physician office staff


Total Parenteral Nutrition (TPN)

  • Nutritional assessments conducted by a registered dietitian
  • Patient follow-up by the Nutrition Support Team to assure safe and appropriate nutrition therapy
  • In-home visits for patient assessment, evaluation and education
  • Individualized plan for patient care and follow-up
  • Comprehensive evaluation of drug interactions by a clinical pharmacist
  • Clinical monitoring of lab values and physician updates
  • Assistance with Certificate of Medical Necessity (CMN) and insurance authorization
  • Consultations with physicians and facility staff
  • Staff education for facilities and physician office staff


Post-Weight Loss Surgery

A variety of nutrition supplements are available for patients who have had bariatric surgery and other procedures. These products can be purchased at GLHHS retail showrooms or from our Web Showroom. For more information, contact diane.harbaugh@hamot.org.

Medical Nutrition Therapy

Medical nutrition therapy at Great Lakes Home Healthcare Services is a one-on-one counseling session conducted by a registered dietitian. Through medical nutrition therapy, we identify interventions helpful during treatment of an illness, injury or disease condition.

The nutrition care process can include assessment, establishment of self-management goals, development of nutrition plan, documentation, communication with the primary care provider, evaluation and reassessment, and outcomes measurements.

Medical nutrition therapy benefits patients who are diagnosed with a serious condition or disease, such as:

  • Severe obesity (children and adults)
  • Bariatric surgery patients
  • Coronary heart disease
  • Hyperlipidemia
  • Hypertension
  • Congestive heart failure
  • Pediatric failure to thrive
  • Gastrointestinal disorders
  • Celiac disease
  • Diabetes*
  • Renal failure*


Please note that nutrition consulting services may not be covered by insurance, unless there is diagnosis of diabetes.
*These patients are consulted by registered dietitians who also are certified diabetes educators.
